Submitting Your Health Screening Results

Once your health screening is complete, you have several convenient options for submitting your results:

• Applicant Portal The fastest and most secure way to submit your results is through the Applicant Portal. Log in to your account and follow the on-screen instructions to upload your documents directly.
• Fax If you prefer to submit by fax, you may send your results to: 704-237-1096
• Email You may also email your results to: dataentry@bchex.io 

Please Note: When submitting via email or fax, avoid including unsecured documents containing Personal Identifying Information (PII) such as your Date of Birth or Social Security Number wherever possible. The Applicant Portal is the recommended submission method as it provides a fully secure transfer of your information.
