CMS Payment and Registration Options for Health Screening Services

Health Screening Registration Options

When registering for health screening services as a Charlotte-Mecklenburg Schools applicant, you will have three options to choose from. Regardless of which option you select, registration and payment are required for all three.


Option 1: Network Facility 

You may choose to have your health screening services completed at one of our network clinics in the Charlotte area.

  • A list of available facilities will be provided during registration.
  • You will be responsible for paying the cost of the services performed at the clinic, as well as the paperwork processing fee.
  • Once registration and payment are complete, you will receive your authorization form to bring to your appointment.

Option 2: Your Own Physician 

If you prefer to use your personal doctor or an outside clinic, you may choose this option instead.

  • You will be responsible for paying the paperwork processing fee only. The cost of your doctor's visit is separate and will be billed directly by your physician's office.
  • Your authorization form and the required NC Schools Health Certificate will be provided to you after registration and payment are complete.
  • ⚠️ Please Note: We do not manage or control how individual physician offices operate. We recommend contacting your doctor's office in advance to confirm whether they are able to complete the required forms without a separate visit, or if an additional appointment will be necessary.

Option 3: Transfer Paperwork from Another NC School or Staffing Agency 

If you have previously completed health screening requirements through another North Carolina school district or staffing agency, you may be eligible to transfer your existing paperwork.

To qualify for this option:

  • You must have prior employment at an NC school or staffing agency
  • There must be no gaps in employment between your previous position and your current CMS application
  • You will be responsible for obtaining the required documents from your former employer and forwarding them to Bchex

You will be responsible for paying the paperwork processing fee only, as no new services need to be performed.


Important Reminder 

For all three options, registration and payment must be completed before any paperwork or authorization forms are issued. Please have a credit card available when you begin the registration process.
