How do I use my authorization form?

Once you place an order for an Occupational Health Service such as a drug test or physical exam, you will receive an Authorization Form (also called Order Confirmation).  Please be sure to bring this with you to the clinic as it is proof of your purchase/payment for the service.  

You will need to set up an appointment with the clinic yourself, as BIB does not have access to clinic scheduling systems.  The clinic contact information and hours of operation are provided on your Authorization Form.  

If the authorization form is expired, the clinic may turn you away, or ask you to pay for it yourself.  If you were not able to complete your appointment prior to the form expiring, please reach out to Bchex Customer Support at applicantinquiry@bib.com.
