Placing a Managed Service Order
To begin a Managed Service order:
From the left navigation panel, select Orders > New Managed Service Order.
The available options may vary depending on your account setup and enabled services.

Create a New Managed Service Order
Complete all required fields on the order form.
Important Notes:
Driver’s License number and state are required for any DOT order.
Employee Position and Other Information fields are optional and may be left blank.

Expected Clinic Visit Date
To the best of your ability, select an Expected Clinic Visit Date.
This date does not determine the applicant’s actual appointment date; however, it does help our system monitor the order status.
If the expected date passes and the order is still open, the system will begin prompting the applicant for updates.
Reason for Service
Select the appropriate Reason for the service being ordered.
Order Specific Notes, Location and Loc Code are all optional.

Job Type and Service Selection
Choose the Job Type first.
Available service options will populate based on the job type selected.
Select the service(s) you need.
Click Choose Providers to view a list of available clinics.
You may update the ZIP Code if you need clinics in a different location.

Choose a Provider
You may choose the same provider for all services, or different ones depending on your requirements. Click "Continue" once you are done.

Complete Order
After selecting a clinic:
The clinic information will display for review.
Click Submit and Email Form to Donor to complete the order.
You also have the option to Submit and Email the form to yourself. Either method will allow you to add multiple recipients to receive the form.

You may add up to 4 additional emails:

Important Ordering Notes
If you do not want the applicant to receive order information (for example, during a Random or Reasonable Suspicion test), enter your own email address on the order. The confirmation details will be sent to you instead.
For drug screen orders, you must always select Save and Email Donor.
The option Submit and Print Auth Form does not provide the clinic with the necessary electronic information to perform the drug screen.Only use Submit and Print Auth Form for physical exams only!
